Enrollment Process

Follow the steps below to make sure your enrollment file is completed as quickly and easily as possible. Once the file is complete the I-Team can communicate about the student

Student Application

The Student Application is an online form that is completed every year for each student, prior to starting I-Team services. The form records basic information about the student and their program to get us started, and also allows you to upload required documents.

Parent Permission

All students need a valid, signed Parent Permission Form (PPF) as part of their file. The form shows that the student’s family is aware that the I-Team will be part of their child’s team and can access relevant records and information.

The Parent Permission Form (PPF) is valid for three years unless the parent revokes permission.  If the student is new to the I-Team, you will need to upload a signed PPF with the online application. If you are not certain if there is a parent permission on file you can check with the I-Team Enrollment Coordinator Caleb Serra.

Consultative Services Agreement

The Consultative Services Agreement form is sent to the Local Education Administrator (LEA) outlining the financial agreement associated with the I-Team services. Once the LEA has signed off on the agreement, the enrollment is complete and everything is filed together electronically. A new Consultative Services Agreement form is signed yearly, prior to the start of any I-Team services.

Student’s IEP and Schedule

The student’s IEP and school schedule are needed as part of a completed enrollment record. There is space provided in the Enrollment Application to upload these documents. If you do not have current versions of either document you can still submit your Enrollment Application and add those in as soon as possible.
