The Student Application Forms section offers links to the documents that are part of the enrollment process. Three main forms are required for a complete enrollment: the Student Application, the Parent Permission, and the Consultative Services Agreement. The Student Application is an online form, described in the section below. The Parent Permission form is printed, signed, and uploaded with the online application. Lastly, the I-Team office sends the school Consultative Services Agreement and receives the signed copy back. More information is available below or in the sidebar.
Student Application
The Student Application is an online form that is completed every year for each student, prior to starting I-Team services. The form records basic information about the student and their program to get us started, and also allows you to upload required documents.

Parent Permission Form
All students need a valid, signed Parent Permission Form (PPF) as part of their file. The form shows that the student’s family is aware that the I-Team will be part of their child’s team and can access relevant records and information.
The permission form is valid for three years unless the parent revokes permission. If the student is new to the I-Team, you will need to upload a signed PPF with the online application. If you are not certain if there is a parent permission on file you can check with the I-Team Enrollment Coordinator Caleb Serra.

Consultative Services Agreement Form
The Consultative Services Agreement form is sent to the Local Education Administrator (LEA) outllining the financial agreement associated with the I-Team services. Once the LEA has signed off on the agreement, the enrollment is complete and everything is filed together electronically. Similar to the Student Application, a new Consultative Services Agreement form is signed yearly, prior to the start of any I-Team services.
