What is Health Care Transition?
Healthcare transition to adulthood is a process. It must start early as youth need to develop skills to manage their own healthcare needs. It also takes some time to find a good match or develop a relationship with a new healthcare provider. Even in adulthood, transitions to new healthcare services and providers might still happen: healthcare providers may change or someone’s needs may change.
We hope the resources found on this website will help Autistic youths and adults, autistic and non-autistic parents and caregivers and healthcare providers to better navigate healthcare transition and the transition to adulthood.
